No regard for quality management and risk analysis

It is important, certainly for larger projects, to talk through and record the decisions on all essential aspects (scope, budget, personnel, deadlines, resources, project organisation etc.) beforehand. A good quality manager and risk analysis, together with all parties involved, can be of immense help. These kinds of activities take time and take up limited project resources but are absolutely necessary for a successful project. A good project manager will take the time, in particular with projects with a tight schedule and limited budget, to look back and apply the lessons from the past to his or her plans for the future.
